Mental Retardation Supports Coordination services at WCSI are designed to assist children and adults with developmental disabilities by locating, coordinating, and monitoring support services.
What is the Role of the MR Supports Coordinator?
Our Supports Coordinators are highly trained professionals who provide a link to available services based on the individual's needs. Some of the activities performed by the Supports Coordinator are:
- Assess the needs of individuals
- Educate the individual and family on service options
- Develop Individual Support Plans (ISP)
- Locate both informal and formal supports and services
- Coordinate supports and services
- Monitor services to ensure satisfaction, health and safety, and that the individual receives appropriate and continuous care
Where Are These Services Provided?
Supports Coordination is provided to individuals living in a variety of settings. For example:
- The individual's home
- Community Homes (Group Homes)
- Personal Care Homes
- Nursing Homes
- Private ICF-MR's
- State Centers
